Alongside Christo Stoichkov and Lyuboslav Penev, he forged a formidable attacking trio that led the team to three Bulgarian Championships and three Bulgarian Cups. Kostadinov also played a pivotal role in CSKA's run to the semi-finals of the 1988/89 UEFA Cup Winners' Cup, where they narrowly lost to eventual champions Barcelona.

He debuted for the senior team in 1988 and gained global recognition in 1994 for scoring two crucial goals against France in the final match of the 1994 FIFA World Cup qualifiers. His second goal, scored in the dying seconds, propelled Bulgaria to the World Cup, denying France a spot in the tournament.
