As a player, Karnehm had greater success in English billiards. He won the National Amateur Championship and World Amateur Championship in 1969. Upon turning professional in 1970, he reached the World English Billiards Championship final in 1971 and 1973, but finished as runner-up on both occasions. In 1980, he finally claimed a professional title, winning the UK Championship by defeating Rex Williams.

Beyond his accomplishments as a player and commentator, Karnehm played a crucial role in developing specialized glasses for snooker. His collaboration led to the creation of glasses with enlarged lenses, which enabled Dennis Taylor to triumph in the 1985 World Snooker Championship.
