As a psychotherapist, Dr. Janov treated patients for sleep disorders, depression, alcoholism, drug addiction, and other ailments. He utilized a range of therapeutic methods, including group therapy sessions with the use of hypnosis.

One day, during such a session, a young man in a trance began to loudly cry out and exhibit oral movements as if sucking from a baby bottle, followed by vomiting. Surprisingly, this intense display significantly improved the psychological state of the young man. This occurrence became the catalyst for Janov's discovery of a new form of therapy, which he named Primal Therapy.

The essence of Primal Therapy lies in the belief that neurosis develops due to the blocking of painful memories experienced during early childhood. Over time, accumulated resentment and the resulting physical and psychological tension lead to emotional and physical pain. This pain is often relieved through self-destructive actions such as aggression, drug abuse, alcoholism, and more, which only worsen the individual's condition.

Janov's Primal Therapy focuses on revisiting traumatic childhood experiences and reliving the pain associated with them. The therapy involves limited intervention, specifically targeting the most significant memories, and utilizes specialized techniques during therapy sessions. The preparatory stage of treatment lasts two to three weeks, during which the patient is isolated from external influences such as books, coffee, cigarettes, and other familiar stress relievers. Daily three-hour therapy sessions are conducted, during which the patient assumes the fetal position, and the therapist guides them back to their childhood memories. At a certain point, the patient begins to cry, make sudden movements, and eventually uncontrollably scream in terror. This state is referred to as the "Primal Scream," after which the therapist begins their intervention. The therapist asks the patient to remember their parents and other family members, engage in conversations with them, and understand that the problems of the past no longer exist and should not impact their present life. Upon completion of this stage, the patient returns to their normal way of life.

The second stage of treatment involves attending group therapy sessions, which are held two to three times a week for a year. These sessions teach deep breathing techniques to help relieve tension and reinforce the achieved results, as well as focus on changing speech patterns associated with past defense mechanisms. After completing these courses, individuals gain emotional balance, develop the ability to objectively evaluate their state in specific situations, and make effective decisions.

Janov documented this methodology in his best-selling book "The Primal Scream." He authored an additional 12 popular books on psychotherapy, with one of them, "Life Before Birth," being nominated for the prestigious Hoffer Award. In 1968, Janov and his wife established the Primal Therapy Institute. During the 1970s, notable individuals such as John Lennon and Steve Jobs sought Janov's therapy. Lennon had initially contacted Janov after reading "The Primal Scream" and requested sessions in England. Although Janov declined, he eventually agreed due to the insistence of Lennon's children, who were avid fans of The Beatles. Sessions took place at Lennon's home, where he was a dedicated patient. However, due to immigration issues, the second stage of treatment could not be completed in Los Angeles when Lennon and his wife visited.

In 1975, Janov's marriage ended, and the divorce proceedings lasted until 1980. After the divorce, he married Françoise Donik, and their son Xavier was born. Vivian, Janov's first wife, continued to lead the Primal Therapy Institute. In 1989, Arthur and Françoise Donik founded the Primal Center, which not only provided psychotherapeutic assistance but also offered professional training.

In the later years of his life, Janov experienced difficulty breathing and problems with his vocal cords. He retired from his practice and moved to Malibu. On October 1, 2017, the renowned psychotherapist peacefully passed away in his sleep.
